29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) Job site, 7203 McKnight Road, Pittsburgh, PA: On or about October 13, 2023, during demolition activities, the employer did not ensure that employees in an aerial lift used appropriate fall protection systems/devices when working in the basket of an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.20(b)(2): The employer did not ensure that a program was in use to perform frequent and regular inspections of the job sites, materials, and equipment made by a competent person: a) Job site, 7203 McKnight Road, Pittsburgh, PA: On or about October 13, 2023, during demolition activities, the employer did not ensure that an appropriate system of hazard identification and abatement was in use at the job site. Deficiencies noted during the inspection included, but were not limited to, unsafe temporary wiring splices/practices, holes in the walking working surface not covered, protective footwear not in use while carrying heavy objects and lack of using fall protection on the building roof and in an aerial lift.
