29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At the job site, employees were not provided eye protection when operating pneumatic nail guns were exposed to eye injuries from flying objects.  The Galvan Villa Ramiro was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1357877, citation number 1, item number 2 and was affirmed as a final order on 12/17/2023, with respect to a workplace located at 1319 Black Walnut, Arlington, Texas .   Went to U.S Treasury collections Assessed :$8,544.14   The Galvan Villa Ramiro was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1361455, citation number 1, item number 1 and was affirmed as a final order on 1/03/2019, with respect to a workplace located at 6616 Elderbury Way, ARGYLE, TX 76226.    Went to U.S Treasury collections Assessed : $48,808.57

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about Oct 12, 2023 and at times prior thereto, at the construction site, employees engaged in residential roofing activities without fall protection were exposed to unprotected falls at approximately 10 feet to the ground below.  The Galvan Villa Ramiro was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number1357877, citation number 1, item number 3 and was affirmed as a final order on 12/17/2018, with respect to a workplace located at 1319 Black Walnut, Arlington, Texas.    In collections for $8,544.14.  The Galvan Villa Ramiro was previously cited for a willful violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1361455, citation number 1, item number 2 and was affirmed as a final order on 01/03/2019), with respect to a workplace located at  6616 Elderbury Way, ARGYLE, TX 76226.
