29 CFR  1926.403(b)(2):  Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  At a worksite located at 2768 11th Street, Lee's Summit, MO, 64081, employees were exposed to electrical shock and fire hazards.  The employer exposed the employees to these hazards by allowing them to connect multiple extension cords together.  This hazard was most recently observed on 10/16/2023.

29 CFR  1926.405(a)(2)(ii)(I):Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:  At a worksite located at 2768 11th Street, Lee's Summit, MO, 64081, employees were exposed to electrical shock and fire hazards.  The employer exposed the employees to these hazards by allowing them to use an electrical extension cord that passed through a pinch point without protecting the cord from damage.  This hazard was most recently observed on 10/16/2023.

29 CFR  1926.405(g)(2)(iv):  Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At a worksite located at 2768 11th Street, Lee's Summit, MO, 64081, employees were exposed to electrical shock hazards.  The employer exposed the employees to these hazards by allowing them to utilize electrical equipment that had strain relief damage.  This hazard was most recently observed on 10/16/2023.  instances of exposure include but are not limited to:  Instance 1:  An electrical extension cord with strain relief damage was in use near the northern section of the worksite to power a radio and power tool battery chargers.  Instance 2:  A Rolair systems air compressor was in use near the northern section of the worksite that had strain relief damage to the power cable.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a worksite located at 2768 11th Street, Lee's Summit, MO, 64081, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing the employees to work at heights greater than 6 feet without the use of fall protection or mitigation measures.  This hazard was most recently observed on 10/16/2023.  Instances of exposure include but are not limited to:   Instance 1:  Employees were exposed to fall hazards due to the potential of falling over the eastern wall and to the exterior ground level below.  Instance 2:  Employees were exposed to fall hazards due to the potential of falling backwards off of the eastern wall to the interior second level of the structure.
