29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: (a) On or before 10/17/2023, the employer did not ensure that employees were protected from fall hazards. Employees were unloading roofing materials on the roof of a new single-family home and were not protected from fall hazards. This condition exposed employees to fall hazards of 20 feet.
