29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about October 12, 2023, in BC-10 Screw Level Production, the employer failed to provide adequate training for employees performing service and/or maintenance activities, to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired. An employee sustained left index, middle and ring finger fractures leading to hospitalization as a result of S5 to S4 conveyor screw not being de-energized and lockout out during servicing and maintenance (unjamming product). Employees were thereby exposed to the hazards associated with moving parts.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a. On or about October 12, 2023, in the BC-10 Screw Level Production, an employee was exposed to machine hazards associated with rotating parts while performing service and maintenance (unjamming of product) on the S5 to S4 conveyor screw, which resulted in left index, middle and ring finger fractures leading to hospitalization. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
