(a) 43'57.863 N, 122"36.522' W-1817 Road, 025 miles from 18 junction, on or about October 13, 2023 and at times thereof: Employer failed to report work-related incident resulting in a in-patient hospitalization for a broken leg requiring surgery to repair as a result of a log striking employee at a fallen tree bucking saw pinched operation.
