29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On October 17, 2023, employees engaged in framing activities without fall protection were exposed to unprotected fall hazards of approximately 11 feet to the ground below.

29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.  On October 17, 2023, the employer did not provide an extension ladder where employees could access and egress the residential building upper levels, exposing them to potential fall hazards.
