On or about October 17, 2023, at the facility located at 838 South 16th Street, Manitowoc, Wisconsin; the employer failed to ensure employees were using a safe means of access and egress when climbing up to the bandsaw table to make adjustments to the guard on the 42" Bandsaw (Model No: R1SE, Serial No.: 10255-RE, Asset No.: 003317), exposing employees to potential fall hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about October 10, 2023, at the facility located at 838 South 16th Street, Manitowoc, Wisconsin; employees were exposed to potential amputation hazards while using a Herschal A-Frame (Serial No.: 5242-0806) knockout machine. Specifically, the moving parts of the hammer were not guarded against incidental contact with employee hands. b. On or about October 17, 2023, at the facility located at 838 South 16th Street, Manitowoc, Wisconsin; employees were exposed to potential amputation hazards while using the Tannewitz Band Saw (Model No.: G1E, Serial No.: 14650-RE). Specifically, the blade guard was set at approximately 11-inches while cutting a casting that measured between approximately 5 inches and 9-1/4 -inches.

On or about October 17, 2023, at the facility located at 838 South 16th Street, Manitowoc, Wisconsin; the air wand used by employees for cleaning purposes on the Tannewitz Band Saw (Model No.: G1E, Serial No.: 14650-RE) was not reduced to 30 p.s.i. due to the pressure reducing tip being removed from the nozzle.
