29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Jobsite: 31 W. Coulter Ave. Philadelphia, PA. - On or about October 17, 2023, the employer did not ensure that each employee conducting pipe installation activities in a trench measuring approximately 20 feet in length, 4 feet in width, and 7 feet 6 inches in depth, were protected from cave-ins by the use of an adequate protective system. Mayfield Site Contractors, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1544408, citation number 2, item number 1 and was affirmed as a final order on 7/3/2023, with respect to a workplace located at 700 Ramona Ave, Philadelphia, PA.
