Location: Worksite; rear of residence.  a) On our about 10/18/2023, the employer did not ensure that the path to ground from circuits, equipment, or enclosures was permanent and continuous. A Bosch Circular Saw and a Bosch PowerBox Subwoofer were both used with missing ground pins in their electrical connectors.

Location: Worksite; rear facing fa�ade of residence.   a) On or about 10/18/2023, an employee worked on a 3-tier high tubular welded frame scaffold that was not fully planked or decked.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access.  Location: Worksite; rear facing fa�ade wall of residence.  a) On or about 10/18/2023, an employee worked on a 3-tier  high tubular welded frame scaffold that did not have a safe means of access. The employee climbed the  scaffold frames and cross braces to climb up and down to access the working platform.

Location: Worksite; rear facing fa�ade wall of residence.  a) On or about 10/18/2023, an employee  was working on the second tier of a 3-tier  high tubular welded frame scaffold was exposed to fall  approximately 15 feet.
