29 CFR 1926.502(d)(23): Personal fall arrest systems were attached to guardrail systems or to hoists: (a) 528 North Queen Street, Lancaster, PA; in the 3-story stairwell under construction closest to North Queen Street - On or about October 18, 2023, employees used personal fall arrest system (PFAS) and a self-retracting lifeline (SRL) which was connected to a Schedule 80 guardrail post, exposing employees up to a 3-story fall to grade. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.95(a):Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: (a) 528 North Queen Street, Lancaster, PA; in the 3-story stairwell under construction closest to North Queen Street - On or about October 18, 2023, employees used a Fall Tech Leading Edge Contractor 50 SPRO self-retracting lifeline (SRL) as part of the personal fall arrest system (PFAS) which was not maintained in a reliable condition in that it was stored by placing it around the guard rail and clipping it back on to the wire. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
