29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: (a) Machine Shop - Edwards 120 Ton Ironworker (SN - 00392191W120): On or about October 19, 2023, an employee operated the hydraulic punch press function of the Ironworker without machine guarding. The employees' hands were within 6 inches of the point of operation exposing the employee to an amputation hazard.
