29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). At a worksite located at 4215 Commonwealth CRT, Saint Joseph, Missouri. The employer, as a general contractor at a multi-employer worksite, did not conduct regular inspections of all operations, equipment, work areas and facilities, to ensure that subcontractors were operating equipment as intended, wearing and using the correct PPE to protect from hazards such as but not limited to fall hazards at the worksite.

29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: At a worksite located at 4215 Commonwealth Crt, Saint Joseph, Missouri. Employees were exposed to an open hole in the roof, measuring 35X31 inches in diameter and approximately 17 feet to the concrete surface below. Absent a cover, guardrail systems, safety net systems, or personal fall arrest systems.
