Location: 207 Beach Road, Salisbury, MA On 10/20/2023, one worker was exposed to injuries from falls when lifted more than 20 feet by a telescopic handler in a job-made box to install a window on the third floor of a residential building.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Location: 207 Beach Road, Salisbury, MA On 10/20/2023, one worker was exposed to injuries from falls when lifted by a telescopic handler more than 20 feet in a job-made box to install a window on the third floor of a residential building. The telescopic handler was not marked and there was no indication weight and balance effects were taken into consideration.
