29 CFR  1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation.  A) On or about October 18, 2023, outside the water tank, where there was a connection to a 12-inch water pipe: An employee was connecting a 12-inch water pipe to a shut off system in an excavation measuring 4 feet deep in which there was accumulated water.  An employee was exposed to crushed-by and struck-by hazards.  B) On October 19, 2023, outside the water tank, where there was a connection to a 12-inch water pipe:  An employee was  using a shovel to backfill the excavation for the new 12-inch water pipe that measured 35 inches deep with the backfill in place in which there was accumulated water from the day before.  An employee was exposed to crushed-by and struck-by hazards.
