29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: (a) Worksite: On or about October 18, 2023, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees using these tools at over 120 PSI engaged in framing and sheathing activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. (a) Worksite: On or about October 18,2023, the employer exposed employees to falls of 20-feet, 5-inches to the lower level, in that complete and adequate fall protection systems were not provided to employees engaged in framing and sheathing activities on a 6/12 pitched roof. Employees were observed working without full body harnesses attached to life-lines or lanyards.
