29 CFR 1910.253(b)(2)(ii):Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. On or about October 24, 2023 employees were exposed to gas cylinders not secured from falling objects or being knocked over.

On or about October 24, 2023, employees utilized equipment with damaged cords in the following locations: The south side used a Miller Delta weld machine with a damaged cord running from the machine to a 240 V power supply.
