29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling. a. On or about September 27, 2023, at Locomotive Railserve 8216 Mod GP-9 at the USD terminal Track A spur, 719 Richard James Avenue, Stroud, OK. 74079, a locomotive was lifted by a sling and crane when employees hand pushed a locomotive truck assembly from the locomotive. This exposed the employees to crush by hazards. b. On or about September 27, 2023, at Locomotive Railserve 8216 Mod GP-9 at the USD terminal Track A spur, 719 Richard James Avenue, Stroud, OK. 74079, an employee was under locomotive traction control motor #3 when a truck assembly was suspended by a sling and crane over traction control motor #3. This exposed the employee to crush by hazards. c. On or about September 27, 2023, at Locomotive Railserve 8216 Mod GP-9 at the USD terminal Track A spur, 719 Richard James Avenue, Stroud, OK. 74079, an employee was under locomotive traction control motor #4 when a truck assembly was suspended by a sling and crane over traction control motor #4. This exposed the employee to crush by hazards.
