29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   Autospot Pavilion: An employee working from the roof of the pavilion canopy to assist in the attachment of aluminum panels to the side of the pavilion canopy was not protected from falling by a guard rail, safety net, or personal fall arrest system. Employees were exposed to fall hazards.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  Scaffold at the base of the arch: Employees were painting from a scaffold that was not fully planked. Employees were exposed to fall hazards.  Noel Manalac was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1671526, citation number 1, item number 1 and was affirmed as a final order on 08-08-2023, with respect to a workplace located at Auto Spot, Tamuning GU 96913.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a. Scaffold at the base of the arch: Crossbraces were used to climb the scaffold to access the work platform. Employees were exposed to fall hazards.  b. Scaffold near the Autospot Pavilion: Crossbraces were used to climb the scaffold to access the pavilion roof. Employees were exposed to fall hazards.  Noel Manalac was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1671526, citation number 1, item number 2 and was affirmed as a final order on 08-08-2023, with respect to a workplace located at Auto Spot, Tamuning GU 96913.

Employees were exposed to fall hazards.
