29 CFR  1910.22(a)(1):   Walking-Working Surfaces - Surface conditions. The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  360 Minor Road: On or about October 26, 2023 & December 28, 2023, Employer did not ensure that Employees were protected from accidental injury because all places of employment, passageways, store rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition. Employer failed to keep operational areas around machines free from debris and trash allowing Employees  to be subjected to tripping-falling hazards. Employer failed to keep production floor clean and organized with established aisles & passageway which would facilitate free and safe movement of personnel and equipment within workplace.  A) Far Section of Facility: Blocked entry/egress points through dye processing area and building exits. B) Center Section - Production Floor: Tripping hazards in and around machinery. Blocked aisles for pedestrian & PIT movement. C) Near Section: Blocked aisles to electrical equipment controls and safety response materials.

29 CFR  1910.22(a)(2):   Walking-Working Surfaces - The floor of each workroom is maintained in a clean and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.   360 Minor Road: On or about October 26, 2023 and December 28, 2023, Employer did not ensure that Employees were protected from accidental injury because every workroom was not not maintained in a clean, and to the extent feasible, in a dry condition. Employer did not maintain or provide dry standing places, false floors, platforms, or mats to facilitate a dry work place.   A) Center of Facility -Production Floor: Initial complaint alleged standing water from roof. B) Far Section of Facility: Standing water from pressurized water discharge lines emptying directly to facility floor in dye processing area.

29 CFR  1910.176(b):   Handling Materials - Secured storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse  360 Minor Road: On or about October 26, 2023 & December 28, 2023, Employer did not ensure that Employees were protected from accidental injury because storage of material created a struck-by hazard. Pallets of corrugated paper were not stacked and/or limited in height so that they were stable and secure against sliding or collapse. Cylindrical cutting dies, some over 6' in length, were stored upright, unsecured, and overhead in areas with unrestricted movement.  A) Far Section of Facility: Pallets of corrugated paper stacked on pallets without binding-banding placed in shelving 2x-3x tiers high, over 15-feet. B) Center Section of Facility - Production Floor: Corrugated paper on pallets, stacked 2x-3x pallets high, over 6-feet high, on the open production floor. Corrugated paper stacked on pallets without binding-banding placed in shelving 2x-3x tiers high over 15-feet. 5-gallon buckets of chemicals stacked in the center of the production floor.  C) Near Section of Facility: Cutting dies 6-feet to 10-feet in length staged upright and unsecured. Cutting dies placed in shelving, unsecured, and over 6- feet high.

29 CFR  1910.303(g)(1):   General - Electrical - Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.  360 Minor Road: On or about October 26, 2023, Employees were not protected of accidental injury because Employer did not provide and maintain sufficient access and working space about all electric equipment to permit ready and safe operation and maintenance of such equipment. 2x sets of respective electrical control panels were partially blocked by storage and debris preventing free access for operation and/or maintenance.       A) 2x Electrical panels mounted on a perimeter wall directly behind facility ventilation system were blocked by spools of wire,       staged cutting dies, and carts of material.       B) Electrical panels mounted in electrical control area of Near section of facility was blocked with trash & debris.
