A) 6001 Bordentown Ave, Sayreville NJ - Building #4 The employer did not ensure that employees isolate hazardous energy sources when unjamming the machine on the Braner USA machine in building # 4, exposing employees to caught in and amputation hazards. Condition occurred on or about 10/18/23.
