This condition exposed employees to uncontrolled hazardous energy. Hazards observed on or about December 24, 2025

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) 6001 Bordentown Ave, Sayreville NJ 08872-Building # 2 /Samco (Serial # 5868, Model # 5868MOS) - The employer failed to ensure that Samco operator shut down the equipment prior to conduct trouble shooting, unjamming roller. This condition exposed employees to amputation hazards. Incident occurred on or about December 24, 2025
