29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer.   On or about October 26, 2023, the employer failed to provide voluntary respirator users who were working around hazardous vapors and fumes produced by laser machines with a copy of Appendix D of the standard.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user.  On or about October 26, 2023, the employer did not establish and implement a respiratory protection program where an employee was provided respirators on a voluntary basis.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about October 26, 2023, the employer did not establish a energy control program for employees who performed servicing and maintenance on embroidery machines, exposing employees to the hazard of being caught in moving parts.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about October 26, 2023, the employer did not develop and implement a written hazard communication program for employees working with and around hazardous chemicals, exposing employees to hazardous chemicals such as, but not limited to Quartz, Molybdenum(VI) Oxide, Stoddard solvent, Ethylene Glycol Monobutyl Ether, and Cellulose Nitrate.  Required program elements that were not developed and implemented include the following: 1910.1200(f): Labels and other forms of warning.  1910.1200(g): Safety data sheets.  1910.1200(h): Employee information and training. 1910.1200(e)(1)(i): Hazardous chemical inventory list.
