Bowling center: On and/or before October 24, 2023, employees conducted cleaning and sanitizing tasks with cleaning products in multiple areas of the facility on an as needed basis. Employer had the following: a) Indoor playground: Preparing and spraying task(s) with Sysco Classic Germicidal Ultra Bleach product was not assessed to determine the presence of hazard(s) which necessitate the use of Personal Protective Equipment(s). b) Kitchen: Hood and grill degreasing task(s) with Keystone Real A-Peel cleaner degreaser aerosol product was not assessed to determine the presence of hazard(s) which necessitate the use of Personal Protective Equipment(s). c) Front Counter: Cleaning and disinfecting task(s) with Keystone Asepticare Aerosol-Disinfectant Virucide product was not assessed to determine the presence of hazard(s) which necessitate the use of Personal Protective Equipment(s). Employees were potentially exposed to eye and skin irritations, redness, and/or contact dermatitis.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Bowling Center: on and/or before October 24, 2023, employees conducted multiple tasks with cleaning and disinfecting chemicals products in various areas of this facility. Employer had the following: a) Indoor Playground: Employees utilized Sysco Classic Germicidal Ultra bleach to clean and disinfect the area and were not provided with eye protection prior to conducting this task. b) Kitchen: Employees utilized Keystone Real A-Peel cleaner degreaser aerosol to degrease the hood and grill surfaces and were not provided with eye protection prior to conducting this task. c) Front Counter: Employees utilized Keystone Asepticare Aerosol Disinfectant Virucide aerosolized spray and were not provided with eye protection prior to conducting this task. Employees were potentially exposed to eye itchiness, redness, and/irritation.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. Kitchen Area: On and/or before October 24, 2023, employees conducted scrubbing and cleaning operation on grill racks with Sysco Reliance Liquid Pot and Pan Detergent on a regular basis. Employer did not ensure that a label with identity, pictograms, hazards, and precautionary statements was affixed to the secondary bottle. This condition potentially exposed employees to eye itchiness, redness, and/or irritation.

Pin setting area/back of the house: On and/or before October 24, 2023, employees conducted emptying, cleaning, and refilling of the AquaGaurd self-contained eyewash tank with water on a biannual basis. Employer did not ensure that the employee(s) were trained on the treatment of the water with bacteriostatic additive, to prevent bacterial growth in the eyewash tank. This condition potentially exposed employees to infection(s) from organisms such as Acanthamoeba, Pseudomonas, and Legionella.
