On or about October 27, 2023, Jose Ojeda DBA Spartan Roofing did not ensure that a portable ladder used to access the roof of a residential building at 1791 Fairview Farms Circle in Wentzville, Missouri, was extended at least 3 feet above the upper landing surface, exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels had not been protected by guardrail systems, safety net system, or personal fall arrest system. On or about October 27, 2023, Jose Ojeda DBA Spartan Roofing did not ensure that each employee roofing a residential building at 1791 Fairview Farms Circle in Wentzville, Missouri, was protected by personal fall arrest systems. Employees were exposed to falls from heights greater than 10 feet. Jose Ojeda DBA Spartan Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1693136, citation number 1, item number 1 and was affirmed as a final order on October 25, 2023, with respect to a workplace located at 524 Meadow Field Lane in Wentzville, Missouri.
