29 CFR 1910.28(b)(5)(i): The employer did not ensure that each employee on a runway or similar walkway is protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system. a) Production Work Area, PD2-I Work Area: On or about October 26, 2023, employees used a walking/working platform that was four feet above grade to process packages along a conveyor belt system but fall protection guarding was not in place.

29 CFR 1910.145(f)(7):Warning tags were not implemented to warn employees of a hazard level between "Caution" or "Danger": (a) Production Work Area, PD2-I Work Area: On or about October 17, 2023, employees were tasked with package handling along the package conveyor belt where the emergency stop buttons were disabled. Warning tags were not implemented to warn employees that the emergency stop buttons were not functional in the event of package jams or other issues that would necessitate the belt to be shut down.
