29 CFR 1910.178(l)(4)(ii): (A) The employer did not provide refresher training to all employees engaged in the unsafe operation and use of forklifts: a.) At the jobsite, located at 2233 Murphy Court North Port, FL 34289; on or about 11/1/23, the employer exposed employees to struck by hazards, in that refresher training was not given to a reach truck operators that were recently involved in a collision with a building column post. The incident occured on or about 10/25/23.
