The employees were observed operating pneumatic nail guns without eye protection and thus exposed to eye hazards.

The employer did not utilize guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection at heights at least as high as 9 feet from ground-to-eave, as measured along the east garage roof. H R Vasquez, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1556628, citation 1, item number 1 and was affirmed as a final order on March, 8, 2022, with respect to a workplace located at 1006 Regency Manor Drive, Ballwin, Missouri.
