29 CFR 1926.250(a)(1): Materials stored in tiers were not stacked, racked, blocked, interlocked, or otherwise secured to prevent sliding, falling, or collapse: On or about October 31, 2023, at 930 NE 1st Avenue, Miami, Florida, an employee was exposed to struck-by and caught-between hazards when, in the process of flagging a formwork that was being hoisted, it struck another formwork that was stored but not secured to an A-frame rack, thus falling from the rack an impacting the employee.
