29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about November 1, 2023, at the above addressed jobsite, an employee performing siding installation operations from a pump jack scaffold, eleven (11) feet six (6) inches above ground, was not protected by guardrails on each end of the platform, thereby exposing the employee to fall hazards. b. On or about November 1, 2023, at the above addressed jobsite, an employee performing siding installation operations from a pump jack scaffold, fifteen (15) feet above ground, was not protected by guardrails on each end of the platform, thereby exposing the employee to fall hazards.
