A) Northumberland Way Monmouth Junction, NJ 08852 - The employer did not ensure to place the excavator at least 2 feet from the edge of excavations. This condition exposed employees to fall hazards and struck-by hazards. Incident occurred on November 2, 2023.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A) Northumberland Way Monmouth Junction, NJ 08852 - The employer did not ensure to place an adequate protective system in the excavation. This condition exposed an employee to cave-in and engulfment hazards at the site. Incident occurred on November 2, 2023. The Underground Utilities Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1468477, citation number 1, item number 1 and was affirmed as a final order on August 27, 2020, with respect to a workplace located at Gotham Parkway and Paterson Plank Road Carlstadt, NJ 07072.
