29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) New residential construction located at Lot # 55 Cool Springs Road, Gainesville, GA 30506. Side of structure: On or about 11/7/2023, the employer exposed employees to fall hazards of approximately 14-feet, 7-inches to the lower ground level, in that a fall protection system was not used while an employee accessed the cantilevered end of the scaffold platform.

29 CFR  1926.451(b)(7): On scaffolds where platforms were overlapped to create a long platform, the overlap did not occur only over supports, and/or was less than 12 inches (30 cm) when the platforms were not nailed together or otherwise restrained to prevent movement.   a) New residential construction located at Lot # 55 Cool Springs Road, Gainesville, GA 30506. Side of structure: On or about 11/07/2023, the employer exposed employees to collapse/fall hazards of approximately 14-feet, 7 inches to the lower ground level, in that the bridged scaffold was not erected on the supporting pole and the scaffold platform was attached using rachet straps which is prohibited by the manufacturer.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) New residential construction located at Lot # 55 Cool Springs Road, Gainesville, GA 30506. Side of structure: On or about 11/7/2023, the employer exposed employees to fall hazards of approximately 14-feet, 7-inches to the lower ground level, in that a fall protection system was not used while working from a pump jack scaffold system used to install siding.
