29 CFR 1926.502(f)(1)(i):When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: On or about September 19, 2022, the employer Gomez Roofing LLC, at jobsite located at 318 W. North St. Lima OH 45801, did not ensure employees were protected from falls by ensuring warning line systems erected around the work area maintained the 6 feet distances between the roof edge and each stanchion and rope. Employees installing an ISO membrane roofing system atop the low sloped commercial roof structure were exposed to a fall of 14 feet and 5 tenths to the ground below.

29 CFR 1926.502(f)(2)(ii):The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. On or about September 19, 2022, the employer Gomez Roofing LLC, at jobsite located at 318 W. North St. Lima OH 45801, failed to ensure the warning line installed around the working area maintained a height of at least 34 inches from the working surface. Employees installing an ISO membrane roofing system atop the low sloped commercial roof structure were exposed to a fall of 14 feet and 5 tenths to the ground below.
