(a): On or about November 8, 2023, at jobsite located at 12880 Illinois Route 47, Huntley, Illinois, the employer did not ensure all employees were wearing a safety harness while working from an aerial lift. Employees had personal fall arrest systems on site but did not utilize them while operating aerial lifts.  BM Construction Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1628038, citation number 1, item number 1 and was affirmed as a final order on January 12, 2023, with respect to a workplace located at 1215 S 1st Ave, Maywood, IL 60153.
