29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 7, 2023, the employer did not provide a training program for employees exposed to a fall hazard of 8.4 feet above concrete while removing and replacing shingles from the roof of a residential home with an 10/12 pitch located at149 Sandstone Ridge Way, BEREA, OH 44017, to minimize these hazards.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:   On or about November 7, 2023, the employer did not ensure employees using air powered nail guns to install shingles on the roof of a residential home located at 149 Sandstone Ridge Way, Berea, OH 44017, were wearing or using appropriate eye or face protection and exposed to struck by hazards.   KEIM CUSTOM EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1658928, citation number 1, item number 1 and was affirmed as a final order on 05/25/2023, with respect to a workplace located at 9165 Old Wallings RD, North Royalton, OH 44133.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about November 7, 2023, employees were exposed to a fall hazard of 8.4 feet while completing roofing work on a residential home located at 149 Sandstone Ridge Way, Berea, OH 44017. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 3 feet above the upper landing surface.  Keim Custom Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1658928, citation number 1, item number 3 and was affirmed as a final order on 05/25/2023, with respect to a workplace located at 9165 Old Wallings RD, North Royalton, OH 44133.
