29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:   On or about 6/11/2024, the employer did not ensure employees using air powered nail guns to install shingles on the roof of a residential home located at 7719 Snow RD, Parma Ohio 44129 , were wearing or using appropriate eye or face protection and exposed to struck by hazards.   KEIM CUSTOM EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1709302, citation number 2, item number 1 and was affirmed as a final order on 12/14/2023, with respect to a workplace located at 149 Sandstone Ridge Way, Berea, OH 44017.  KEIM CUSTOM EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1658928, citation number 1, item number 1 and was affirmed as a final order on 05/25/2023, with respect to a workplace located at 9165 Old Wallings RD, North Royalton, OH 44133.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 11, 2024, employees engaged in removing and replacing shingles on the roof of a residential home located at 7719 Snow RD, Parma Ohio 44129, 11.2 feet above concrete with a 8/12 pitch were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.   KEIM CUSTOM EXTERIORS, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1709302 citation number 2, item number 2 and was affirmed as a final order on 5/25/2023, with respect to a workplace located at 149 Sandstone Ridge Way, Berea, OH 44017.
