29 CFR 1910.22(a)(1): 29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) Receiving and Shipping Area- On or about November 7, 2023, and at times prior, the employer exposed employees to tripping and struck by hazards in that, the employer failed to ensure boxes of merchandise were clear from the warehouse aisles.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE).  If such hazards were present, or likely to be present, the employer did not: (i) Select, and have each affected employee use, the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment; (ii) Communicate selection decisions to each affected employee; and (iii) Select PPE that properly fit each affected employee:  a) Outside shipping and receiving area, on or about November 7, 2023, and at times prior, the employer exposed employees to chemical burns, in that the employer did not provide appropriate personal protection equipment (PPE) for the body when changing propane tanks.

29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Outside shipping and receiving area; on or about November 7, 2023, employees changing out the propane tanks were exposed to chemical burns, in that the employer did not provide the correct face protection.
