29 CFR 1910.37(b)(5):Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet). On or about November 7, 2023, personnel doors which are part of the aircraft hangar rolling door system were used as an exit and exit discharge during an emergency. The personnel doors were not properly illuminated and marked with signage used to notify employees and others accessing the area that the doors were "Not an Exit" was not attached to the doors thus potentially exposing employees to smoke and/or other fire hazards.

29 CFR 1910.157(e)(3):The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request. On or about November 7, 2023, the employer did not ensure that annual maintenance checks of portable fire extinguishers located throughout the facility were completed as required. Records with the name of the individual(s) who conducted the inspection and along with the date the inspection was completed were not retained and/or made available for review by a representative of the Secretary of Laborer.
