29 CFR  1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation:  (a)Smithfield Packaged Meats Corp. at 1400 North Weber Avenue, Sioux Falls, SD 57103:  On or about and at times prior to November 8, 2023,  the employer did not ensure that pallet jack operators were trained on specific operations including how to safely travel through congested areas where access is controlled via an overhead door with an automatically timed closure.  This condition exposed employees to crushing, caught-in and struck-by hazards between the powered pallet jack and the closing overhead door.
