29 CFR  1926.651(i)(3):Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:  At a commercial worksite located at Floral Ave & N Belt Hwy, St. Joseph MO -- employees were exposed to crush-by hazards in that the pavement of the road had been undermined with no cave-in protection. Additionally, there were no metal plates on the roadway to assist in evenly distributing the weight as vehicles passed by. This was documented on or about 11/14/2023.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  At a commercial worksite located at N Belt Hwy & Floral Ave in St. Joseph, MO -- employees engaged in trenching/excavation for water/sewer lines working in depth greater than 5 feet were not protected with cave-in protection. This exposed the employee(s) in the trench to crush-by hazards of ground dirt/debris. The trench was on the outside south lane of the Belt Hwy and East lane of Floral Ave. This was documented on or about 11/14/2023.  Rodriguez Mechanical Contractors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1533443, citation number 1, item number 2(b) and was affirmed as a final order on 11/22/2021, with respect to a workplace located at 103 Hickman Rd, KC MO 64137.
