29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. At the job site located at 17th St. & Walnut St., Kansas City, MO 64108, employees were exposed to slip, trip, and engulfment hazards due to the employer not providing a safe means of egress from an excavation. Employees were working in an excavation approximately 4 feet 7 inches deep. The only means of egress was a raised portion of the excavation and a concrete block being used as a step to enter and exit the site.
