Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, including severe bums, to employees in that employees were exposed to fire, flash fire, deflagration, or explosion hazards from unvented dust collectors located inside a building.  Production Area:  Dust collectors including, but not limited to those listed below, were not equipped with explosion protection system and were located inside the facility.  (a) Storage/Electrical Equipment Room - two Dantherm (Nederman) dust collectors that collected combustible fiberglass resin dust from the Robot (cutting) Room were not equipped with an explosion protection system and were located inside the room used for storage adjacent to the Robot (cutting) Room.  (b) Cutting Area - left side Donaldson Torit dust collector that collected combustible fiberglass resin dust from the Cutting Booth was not equipped with an explosion protection system and was located inside the production area.  (c) Cutting Area - right side Donaldson Torit dust collector that collected combustible fiberglass resin dust from the Cutting Booth was not equipped with an explosion protection system and was located inside the production area.

Production Area:  Where combustible dust generated by the manufacturing of fiberglass products was present, the work area was not kept clean.  a) Robot Room - The area where fiberglass cutting was performed had explosible fiberglass dust located on the ventilation ducts near the ceiling, electrical raceways and pneumatic pipes along the walls, wall ledges and process equipment on or about November 29, 2023.  b) Storage Room adjacent to the Robot Room - The area where fiberglass cutting was performed had explosible fiberglass dust located on the electrical equipment such as 480-volt circuit panels, raceways along the walls, two dust collectors for the Robot Rooms, and other process and general use electrical equipment and work surfaces on or about November 29, 2023.  c) Cutting Booth - The area where fiberglass cutting was performed had explosible fiberglass dust located on the electrical raceways along the walls, process equipment and work surfaces on or about November 29, 2023.
