29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer was failing to protect employees from caught between and amputation hazards. This was most recently document on November 16, 2023 at Wholestone Farms located at 900 South Platte Avenue, Fremont, Nebraska in the Rib Conversion Area. The employer failed to ensure that authorized employee(s) isolated the hazardous stored energy prior to cleaning the T0134 Double Overhead Conveyor. The employer did not require employee(s) to affix a lock to the electrical lock out switch on the conveyor as required by this subpart.
