29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 16, 2023, employees conducting roofing work on a first-floor roof with a 4:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9) feet on the front / North side of the roof. (b)On or about November 16, 2023, employees conducting roofing work on a Second-floor roof with a 6:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 17 feet on the West side of the roof. (c) On or about November 16, 2023, employees conducting roofing work on a detached garage roof with a 4:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately seven (7) feet, six (6) inches to 11 feet, eight 8 inches on the front / North side of the roof. MF Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1622142, citation number 1, item number 2 and was affirmed as a final order on February 14, 2023, with respect to a workplace located at 2 Falcon Lakes Dr. South Barrington, IL. 60010.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On or about November 16, 2023, a Werner extension ladder used by employees to access and egress the detached garage roof did not have its side rails extended at least 3 feet above the roof eave. This exposed employees to a fall hazard of approximately 10 feet onto asphalt. MF Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1)), which was contained in OSHA inspection number1622142, citation number 1, item number 3 and was affirmed as a final order on February 14, 2023, with respect to a workplace located at 2 Falcon Lakes Dr. South Barrington, IL. 60010.
