29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)   (a) On or about November 16, 2023, the employer did not ensure the competent person was conducting frequent and regular inspections of the job site, to ensure fall protection was being worn and tied-in,  that ladders were installed properly, and job site conditions were maintained safe.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about November 16, 2023, employees conducting roofing work to include stripping and prepping a 2:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9) foot, two inches on the south side of the first floor roof.   (b) On or about November 16, 2023, employees conducting roofing work to include stripping and prepping  a 6:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 22 feet from the South side of the second floor roof ridge.  MF Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1622142, citation number 1, item number 2 and was affirmed as a final order on February 14, 2023, with respect to a workplace located at 2 Falcon Lakes Dr. South Barrington, IL. 60010.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) On or about November 16, 2023, a multi-position extension ladder used by employees to access and egress the  roof did not have its side rails extended at least 3 feet above the roof eave. This exposed employees to a fall hazard of approximately nine (9) foot onto grass, gravel and dirt.   MF Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1926.1053(b)(1)), which was contained in OSHA inspection number1622142, citation number 1, item number 3 and was affirmed as a final order on February 14, 2023, with respect to a workplace located at 2 Falcon Lakes Dr. South Barrington, IL. 60010
