29 CFR  1910.38(c)(2):The emergency action plan did not include procedures for emergency evacuation, including type of evacuation and exit route assignments:  a) Geodis  - located at 651 Boulder Dr., Breinigsville, PA - Carbon Dioxide area - On or about November 16, 2023, the employer did not have evacuation procedures outlining safe and effective evacuation from an uncontrolled carbon dioxide breech and/or leak exposing employees that cross through affected areas to the hazards of carbon dioxide.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR  1910.38(f)(1): The employer did not review the emergency action plan with each employee covered by the plan when  the employee was assigned initially to a job:  Geodis - located at 651 Boulder Dr, Breinigsville, PA - Carbon Dioxide area - On or about November 16, 2023, the employer did not fully train all employees on evacuation procedures in the event that an uncontrolled carbon dioxide breech and/or leak would occur, where employees would evacuate through the doors closest to the carbon dioxide area exposing them to carbon dioxide hazards.
