29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) At 671 S Perry St, Montgomery, AL (Water Utility Vault - Trench): On or about November 16, 2023, the employer exposed employees to entrapment hazards in that a safe means of egress such as but not limited to a ladder was not provided to employees performing water utility operations in a trench that was approximately, 4 feet 11 inches (1.50m) deep. Murphy Excavation, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.651(c)(2)), which was contained in OSHA inspection number 1383889, citation number 1, item number 2 and was affirmed as a final order on April 24, 2019, with respect to a workplace located at the Corner of Northern Blvd and Contractor Drive, Montgomery AL.
