29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide abatement documentation for one citation issued April 12, 2024, with citation 1-1 related to providing required fall protection safety equipment.

29 CFR  1926.501(b)(11):  "Steep roofs."  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems.  a)  On or about November 17, 2023, at 3402 N Richmond Street in Appleton, Wisconsin, building's southern end, employees were exposed to a fall hazard working on a roof 8 feet above surface level without using fall protection equipment.  b)  On or about November 17, 2023, at 3402 N Richmond Street in Appleton, Wisconsin, building's western side, an employee was exposed to a fall hazard working on a roof 10 feet above surface level without using fall protection equipment.  c)  On or about November 17, 2023, at 3402 N Richmond Street in Appleton, Wisconsin, building's northern end, employees were exposed to a fall hazard working on a roof 10.5 feet above surface level without using fall protection equipment.

29 CFR  1926.503(a)(1):  The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about November 17, 2023, at 3402 N Richmond Street in Appleton, Wisconsin, the employer did not provide required fall protection safety training for employees working at heights greater than six feet.
