29 CFR 1903.19(d)(1):  The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer has not certified to OSHA, and provided documentation, that the hazardous conditions identified in citations 1-1 and 1-2 of this inspection, which was issued on September 30, 2024, have been corrected including the methods taken to correct the hazardous conditions.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  800 Roof Pro LLC does not require the use of fall protection during roofing operations.  This violation was most recently observed on June 13, 2024, at a commercial project located at 252 Wisconsin Street, Oshkosh, Wisconsin.  Employees were exposed to fall hazards of 9 feet 8 inches to 12 feet 9 inches while installing new asphalt shingles without any form of fall protection.  800 Roof Pro LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1711196, citation number 1, item number 1 and was affirmed as a final order on June 3, 2024, with respect to a workplace located at 3402 N Richmond Street, in Appleton, Wisconsin.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  800 Roof Pro LLC does not provide training to employees exposed to fall hazards.  This violation was most recently observed on June 13, 2024, at a commercial project located at 252 Wisconsin Street, Oshkosh, Wisconsin.
