On or about November 20th, 2023, at the jobsite, employees were exposed to fall hazards, in that fall protection systems were not inspected prior to each us. The rope used for fall protection was worn and frayed and was inspected once every three months.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). a. On or about November 20th, 2023, at the jobsite, employees were exposed to approximate 19 foot fall hazard while working on the second story roof of a home, in that fall protection was not used. The Orion Builders Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1534866, citation number 1, item number 1 and was affirmed as a final order on 10/25/2021, with respect to a workplace located at Lot 224 Red Tail Blvd Sorrento, FL 32776.
